1. Please attach an original certificate of membership from your previous medical insurance, to your application form if:

• Your Membership period was more than two years
• The period between your break from previous medical aid to application for joining Bomaid is no more than three months

Should you need to make amendments to your membership details (e.g. change of surname, banking details, address etc.), please complete a Member Information Update form.

Single male applicants who wish to register a child born out of wedlock must complete an application form and supply the following additional information for their child:

• Application form
• Abridged birth certificate
• Sworn Affidavit* (*only required if the father’s name does not appear on the birth certificate)
